You can now search online---from inside our medical center or from your
home----and find a rehabilitation facility or a skilled or assisted-living
accommodation that meets your needs. You specify your needs through an easy
selection tool and the program does the rest, matching your needs and even
listing which facilities have an opening!
St. Francis wants to make the transition from our hospital walls as
smooth and stress-free as possible.
“My Care Report” is another useful tool for finding a facility that meets your
needs. This tool uses Care Interpreter software and asks you a few
questions regarding the person you are looking for:
finances
health status
memory impairment
independence
special considerations
In most cases, this takes 5 to 10 minutes to complete.
Based on these answers, an easy to understand grid is created that
outlines:
The different assisted care settings available, including providing care
at home
How well matched each setting is to your specific needs and situation
Detailed information and advice helping you learn more about your
options
