In South Carolina, patients legally can let their families and health care professionals know their intentions if they are unable to speak for themselves by using advance directives called living wills and health care powers of attorney.

All patients are encouraged to complete an advance directive prior to entering the hospital, and bring a copy of this document with them to be placed on the medical record. The living will and health care power of attorney forms are available in both English and Spanish, free of charge, from the South Carolina Hospital Association, below as a PDF download,or by calling 864-255-1093.
