How To Schedule Your Appointment
Your physician must refer you for a mammogram. He or she will give you a physician order form, also know as the Mammogram Referral Form, for you to bring to your appointment.
Call 864-255-1396 to schedule the date and time of your appointment. If you like, you may also pre-register over the phone or via the online pre-registration form. This will save some time with paperwork when you arrive for your appointment. When You Schedule Your Appointment
